Program Description: Learn to provide nursing and health care to patients and clients in the community
through a fully online interactive continuing education simulation that takes place
in the home health setting. This virtual case study helps prepare both current and
future nurses to care for clients that includes a focused patient assessment, a home
health assessment, a review of medications, and intervention during a patient/medical
emergency.

Who Should Attend: Nurses, People providing home health care, and Nursing Students
Learning Objectives and Outcomes: Upon completion of this activity, participants will:
- Enhance or gain knowledge of Social Determinants of Health and screening and assessment
tools that can be utilized in the home and community settings.
- Be better prepared to provide care to patients and clients in the home or community
setting.
