This form is for use by current CCRI Driver Ed Instructors only.

Please submit your request for approval with the following information.

Please Note:

  • Please submit one request for each class.
  • All requests must be received 40 days in advance of the start-date of your class. Any request not received within 40 days of the start date, will not be approved.
  • By submitting this form, you are confirming your availability to teach on the days and times indicated and the accuracy of the information provided.
