    DSS Department of Corrections Policies & Procedures 

    Policies 

    • Through the CCRI Campus Coordinator, CCRI students who request accommodations for courses at the Department of Corrections (DOC) may be eligible for reasonable accommodations. 
    • Students must meet the following criteria: 1) be enrolled, 2) self-disclose, 3) be evaluated. 
    • DSS must consult with the appropriate officials at the Department of Corrections (DOC) verification of disability. 
    • Approval for accommodations must be coordinated by the CCRI Campus Coordinator, faculty, student and DSS. 

    Procedures 

    • Campus Coordinator will disseminate DSS information prior to the start of the semester and DOC instructors will include the DSS notation in their syllabus under Resources for Teaching/Syllabus). 
    • Student will make a request for accommodation known to CCRI Campus Coordinator, who will complete the Intake Form and mail/email to DSS, preferably 4 weeks before the start of the semester (Will Jackson: 401-462-2538, [email protected]). 
    • DSS will contact the appropriate DOC office to review the Verification of Disability Form via phone and send form by email. The DOC office will complete the form and email to DSS. (Behavioral Health, Dr. Cerbo, 401-462-0432; ADA Coordinator, Joseph Moroco, 462-3792; Mental Health Clinician, Caitlin Bouchard, 462-3348). 
    • DSS will contact the faculty via email to request a copy of the syllabus, discuss the accommodation and ask that a conversation be scheduled between the student and faculty to review the implementation of the accommodation. 
    • DSS will prepare the Request for Accommodation (RFA) letter, email to Campus Coordinator to obtain student and instructor signature, and mail/email the RFA back to DSS. 
    • DSS will visit the student for the File Face Sheet discussion to review Student & DSS rights and responsibilities. Multiple RFAs may be signed at this time to improve the efficiency of continuing the process, if the student indicates intent to take classes in the future. The signed RFAs will be emailed to the faculty for their signature and returned to DSS for continuing semesters. 
    • An informal follow up by the Campus Coordinator with the student and faculty at mid-term is recommended. 
    • DSS will maintain a student file.
