Employees and applicants have the right under state law to request a reasonable accommodation
for considerations related to pregnancy, childbirth, menopause and related conditions.
To submit a request for a reasonable accommodation, an employee must submit a Disability
Verification Form and Reasonable Accommodation Request form to The Office of Civil
Rights & Title IX.
