Remote ACCUPLACER Request Form

Note: You must have access to a desktop computer or laptop with a camera and microphone in a private setting (public spaces like a Library cannot be used) 

Complete all sections of the ACCUPLACER Registration Form below.  Once completed, you will receive an email from Accuplacer with instructions on setting up your remote test.  Please submit ONLY ONE request at a time for the ACCUPLACER Voucher.

*Important -  CCRI uses a third party, Examity, to proctor the ACCUPLACER remote test. Read Examity's Privacy & Security policy HERE. Examity requires students to show their photo ID via webcam before taking the ACCUPLACER test. The name on your photo ID (license, state ID, student ID, passport) must match the name you gave CCRI in order to schedule your test.  If you have changed your name or have multiple last names on your ID but only gave CCRI one, please update your record with the Office of Enrollment Services before completing this form.
