To view the actual documentation required to complete your file, follow the 3 steps listed in the above tutorial, then select the Eligibility link under Financial Aid, then select the Student Requirements link.

Form Descriptions

  1. Financial Aid Verification Worksheet (Dependent Student): The Financial Aid Verification Worksheet verifies information about the family size, Untaxed income, and other tax schedule information from the FAFSA. This form is required for all dependent students selected for verification by the United States Department of Education.

Financial Aid Verification Worksheet (Independent Student): The Financial Aid Verification Worksheet verifies information about the family size, Untaxed income, and other tax schedule information from the FAFSA. This form is required for all independent students selected for verification by the United States Department of Education.

  3. Asset Verification Form: The Asset Verification forms collects asset values based on information provided on federal tax returns.

  4. Re-Evaluation of Income worksheet:  The Re-Evaluation of Income form is used to provide a student with an opportunity to inform our office of changes in income after the FAFSA has been filed. Those changes include, but are not limited to, loss of employment, parent loss of employment (for a dependent student), or any other extenuating circumstances that result in a significant decrease in either taxable or untaxed income.

    This form is not required for all students. Students needing this form will generally be referred to it by an Enrollment Services Officer.

  5. Financial Aid Appeal Form:  The Federal regulations governing the awarding of Federal Financial Aid requires that a student be making Satisfactory Academic Progress. In the event that you fail to meet the requirements, you may appeal. You need to use this form to submit this appeal.

  6. Financial Aid Dependency Status Review Form:  The Federal government has strict guidelines for a student to be considered independent for Financial Aid purposes. If you do not meet the automatic criteria to qualify as an independent student, but you feel that you are independent and should be considered as such for aid purposes, you may submit the Dependency Status Review form, and all the required additional documentation to support your claim, to our office.
